    Together with a completed and signed International Student application form, all students are required to provide the following supporting documents for Massey to assess student eligiblity:

Evidence of citizenship (birth certificate or current passport) High School graduation certificate Academic transcripts with grading scale for each semester of study at any institution you have studied at Evidence of English Language proficiency Course outlines, if applying for credit assessment Portfolio or audition tape, if applying for a Design or Fine Arts programme Evidence of any scholarships you have been awarded for your intended study at Massey University A statement of any disability or health condition from a health professional, if applicable     1.公民身份證明（當前護照出生證明）          2.高中畢業(yè)證          3.曾經(jīng)就讀的大學的每個學期的成績單          4.英語水平證書          5.若申請學分評估，需列舉所學課程          6.如申請設(shè)計或美術(shù)專業(yè)，請準備自己的作品          7、獎學金證書          8、醫(yī)院專業(yè)人員提供的身體狀況與健康狀況陳述
